Why a vial price and a monthly price cannot be compared

A 15 mg vial at $80 sounds cheaper than $149 a month, and it might be twice the price. At 500 mcg nightly that vial is thirty doses and runs out in a month. At 200 mcg it is seventy-five doses and lasts most of a quarter. Same vial, same $80, and a threefold difference in what a month costs — decided by a number the seller leaves to a prescriber and never prints beside the price.

That is not a criticism of pricing by the vial. It is an honest way to sell a drug whose dose varies. It simply means the figure on the page is not comparable to the figure on the next page, and no amount of staring at the two will make it so.

The part the arithmetic cannot reach

Three things move the real answer and none of them is published. Dead volume — the liquid left in the vial neck and the syringe hub — means the last dose is often not drawable. Overfill runs the other way: pharmacies commonly put slightly more in the vial than the label says. And a reconstituted peptide has a beyond-use date, so a vial that lasts fifty days on paper may not be usable for fifty days.
